Forms and Documents Required:
All forms and fees must be received by Gymnastics Ontario no later than Monday, May 23, 2022.Payment and registration must be done by the club through the online Registration Management System (RMS) - Uplifter, and on KSIS Platform
Uplifter registration: Clubs can login to register, remit payment and order tracksuits using the same account that they use to register their participants for GO Memberships.
All athletes and personal coaches must register. Registration includes:
- Registration in Uplifter
- Payment via Uplifter (club account)
- KSIS registration
Forms:
Clubs are responsible for collecting all of the forms and payments from parents. All forms must be scanned and uploaded to GymCan here
- Waivers (Consent & Assumption of risks) are available here
- Please note that since this event is not held in person, the medical forms are not required.
