• Tracksuit Jacket order (if needed)

Forms:

Clubs are responsible for collecting all of the forms and payments from parents. All forms must be scanned and uploaded into GO Dropbox

• GO Code of Conduct
• GO Medical Data Form
• Travel Consent Form * must be notarized and original given to the travelling team manager
• Copy of Passport (must be able to see photo clearly)
• Copy of Out of Country Heath Insurance

• GO Code of Conduct
• Team Coach/Manager - Roles and Responsibilities Agreement 
• Copy of Passport (must be able to see photo clearly) 
• Police Check (valid within 1 year of competition) (online check option - mybackcheck)

Team Ontario Apparel:

All travelling team members (including coaches) will be required to to have the current G.O. tracksuit.

Team Ontario Tracksuits ($65 + HST)
Clubs must purchase tracksuit jackets online under our Purchase Products Page with the individual's registration checkout. Pants/leggings are not included therefore athletes are required to get/can use their own pants provided they are solid black (no logo/designs) in addition to the jacket.

Music:

All music must be emailed to RG Program Manager at rhythmic@gymnasticsontario.ca. Only MP3 files will be accepted. All athletes must also bring a set of CD's to the event as a backup. 

Technical Program:

Junior athletes will be registered in Level A

Novice athletes will be registered in Level A

Provincial athlete may choose to be registered in Level A or Level B
